
Patients are now able to apply for access online for the following services:
- Repeat Prescription ordering
- Appointment booking and cancellation
- Viewing your health records
In order to use this service you must register first; please read the information below.
How to Register
You must be registered with the practice
We will then post to you your log-on details as part of our verification process.
Either ask our reception team for an information leaflet and application form, or download these here
Your completed application form will need to be returned in person with the required ID. Two documents from the following list, one of which must be picture ID in order that our reception team can compare the applicant to the image, must be provided:
- Passport
- Driving Licence
- Bank Statement
If you are already registered for online services (1 and 2 above), you will not automatically have access to view your health records as well. You will also have to go through the above verification process.
